Information for Patients

On your first visit please remember to bring the following:

  • Patient Information Form (if you are unable to print this out, please don’t worry, you can complete it when you arrive.)
  • Referral letter from your GP or Specialist. Your referring doctor may have sent this directly. Please contact us if you are unsure.
  • Medicare card and Private health fund details.

In addition, it is important to provide Dr Baruah with any relevant medical history to ensure the optimal management of your condition. Items that may be relevant include:

  • List of current medications
  • List of any allergies
  • Previous MRI or CT scans relating to gynaecology
  • Copies of pathology reports and ultrasound scans

What to expect on your first visit:

During your first consultation, Dr Baruah will be asking for detailed information covering your medical and gynaecological history. Accuracy is important as it provides the information needed to assess and properly treat you.

At your appointment, Dr Baruah may perform a physical examination and take some histological samples or a CST (PAP smear)

Physical examinations can at first seem daunting, but there will be a nurse or chaperone in attendance and Dr Baruah will explain everything as he goes and help you feel supported and comfortable during this process.
